How do I know if I'm eligible?

Step 1:

Government-funded in-home support begins with My Aged Care. Call 1800 200 422, apply online through My Aged Care, ask your GP, health professional or hospital to refer you, or book an appointment with an aged care specialist officer at Services Australia.

Step 2:

If eligible, you’ll meet with an aged-care assessor who uses the Integrated Assessment Tool (IAT) to understand your goals, health, function, and circumstances. Together, you’ll discuss what supports could help you stay independent.

Step 3:

Receive your support plan and Notice of Decision

After your assessment, you’ll receive:

A support plan outlining agreed goals and recommended services, and a Notice of Decision, confirming your eligibility and approved support level.

If approved for funded services, you’ll receive a referral code to share with your chosen provider.

Step 4:

Choose a provider.

Supports are adjusted as your needs evolve. Open communication with our care coordinators ensure you always receive the appropriate level of support.

Can I choose my services?

Absolutely. You inform or select the services that are most important to you, ensuring you receive the care that best supports your independence.
